I am having trouble adding a subdomain, I have a domain name registered on Cloudflare and my domain hosted on dreamhost - I am not sure where to make the changes advised below, should it be on Cloudflare or Dreamhost. And assuming it is just Cname I need to do.

It sounds like your domain is controlled through cloudflare. With Pickaxe, we host the studio page for you. So if you’d like to add a subdomain, don’t worry about dreamhost. Just add the CNAME listed to the DNS record in cloudflare.
It looks from your screenshot like you’re not trying to add a subdomain, but instead a root domain. In that case, you’d be putting your Pickaxe studio at the core of your website. If that’s what you’d like to do, instead you should use an A record to get the job done.
