Feedback form for users

Hi all,
I want to collect user feedback on my AI Agents performance.
At the end of a sequence of prompts by the user (in one AI Agent), I’d like to display a ratings or form UI components so the user can complete / input feedback based on their Assistant Experience.

OR…
Is it possible to (upon user selection {when_user_completes_query}, prompt or link to a feedback Agent and get the experience feedback through the separate Agent?

Personally, I use my mini site to embed the pickaxe and a feedback form to capture the information requested.

I recommend sleekplan for an onsite widget, i find it helps a lot!
