Are you asking how to create a chatbot that asks questions sequentially? Are asking whether you can create a form that kicks out into a chatbot?
If you want your chatbot to start as a form, you can create a Smart Form, and then on the control panel you can toggle on “enable chat responses” setting for that Pickaxe. When enabled, this setting allows users to open up the response from a form as a conversation in a chatbot window. But currently there is no way to make a form automatically kick the user out into a chat experience.
